In choosing your primary care physician, you have a designated Care Manager available to you that can help assist you with any of the following services.

If you find you need assistance with any of these and are not sure which Care Manager is yours, please send email to: referrals@genesisdocs.org with your name, number and who your primary physician is, and we will have your assigned Care Manager call you.
